Disability Determinations

Disability determination examinations are conducted by referral only through Louisiana Disability Determination Services. Claimants do not schedule directly with our office. If you have a scheduled examination, your appointment was arranged through DDS and you will receive scheduling information directly from them. What to Expect at Your Appointment 
No administrative paperwork. You will not be asked to complete any intake forms or paperwork before your appointment. Simply bring a valid photo ID and arrive no more than 10 minutes early.

Your appointment will include a clinical intake interview followed by an examination with your assigned provider. Depending on the nature of your claim, your examination may also include a brief neurocognitive assessment. Your provider will explain any assessment before it begins.
